Protected Patient health information obtained by Newport Health Network will be viewed and utilized exclusively by employees of Newport Health Network solely for the purpose of providing quality hearing and balance healthcare.  Protected patient health information will never be released, except for the purpose of treatment, insurance billing, and healthcare operations.  It is our privacy policy that no other disclosures of your health care information will be made without prior written authorization, except as required by law.

The hearing device is new and warranted against defects in material and workmanship for a period of 2 year(s) from the date of purchase. Remakes, however, are warranted for one year only, and earmolds are warranted for 90 days only. In the case of loss or damage during the warranty period, a one-time replacement will be provided, subject to a deductible of up to $200 per aid (plus the cost of a new earmold, if applicable).
